Library Card ID is your library card number (including the hyphen, OR just the last 5 digits of the number).
PIN is no longer your old password you created. Instead you will need to use the password that you use to log in to your library card account. The default password is your phone number (10 digits, no dashes), unless you have changed it to something else.
